Jarrod Shapiro, DPM reviews the fundamental concepts of medical documentation. Specific documents discussed include: the consultation note / history and physical, operative report, admitting orders, operative note, follow-up SOAP note, and discharge summary. This is done through a case presentation format with downloadable templates provided for the viewer's education. Topics also discussed include methods to prevent medical errors through appropriate charting and basic chart rules.

Steve McClain, MD helps you better differentiate a nevus from a melanoma. Dr McClain presents numerous slides of lesions and discusses how to determine whether it is a nevus or melanoma. He reviews the criteria for diagnosis of melanoma and discusses when a biopsy should be performed.
