As was explained by Codingline (as I recall).  You are making a decision to or not to do surgery.  You are taking radiographs and using your training to decide the course of treatment.  Place the 57 modifier next to your E&M then bill the appropriate fracture care code 284--.  Do this along with whatever radiographs or DME included in the visit.
